Report Your Injury immediately.  Failure to report your injury to your employer within 30 days may
jeopardize your ability to receive workers’ compensation benefits.

WORK RELATED INJURIES:

  1. Visit the building nurse to evaluate the injury.
  2. Complete Employee Injury Report (form provided below).
  3. Contact District Workers' Compensation Coordinator listed above for all injuries requiring additional treatment by an authorized physician with the district's workers' compensation insurance.

 

All injuries requiring an ambulance or treatment at an emergency room are to be reported immediately to the WC Coordinator/Superintendent.


TREATMENT FACILITIES:
The medical providers listed below are the designated medical providers for the district.  If an employee's work-related injury requires additional medical attention, the employee, building nurse, or employee's supervisor is to contact the district's Workers' Comp Coordinator in order to obtain a treatment authorization form.  If an employee chooses to use his/her own provider, the employee may be responsible for all cost associated with the services received.
