​Proof of Residency  
 
REQUIRED ENROLLMENT DOCUMENTATION

Property Owner/Parent/Legal Guardian shall provide the following:

1. Parent/Legal Guardian picture identification (current driver's license).
2. Child's birth certificate and immunization records.
3. Current proof of residency: one element required
    a. Deed, real estate property tax receipt, payment book or recent sales contract.
    b. Rental Property — Copy of lease agreement.
    c. Two current credit billings at that address (insurance, credit card billing, utility billing, etc.) or personal
property tax receipt.
    d. Employer's record of that address (check stub with address on it or letter from employer verifying
employee's address).
    e. If not employed, governmental acknowledgment of that address, i.e., Social Security, welfare,
voter registration, etc.
4. Court documents custody/guardianship papers, if applicable.
5. Past school records (if applicable).
    a. Discipline records
    b. Attendance records
    c. Transcript of credits

St. Charles R-5 School District may require additional items or
information necessary to make a final decision on proof of residency.
